Changing Name After Marriage 

Social Security has a rule that brides must file an SS-5 form (a new application showing change of name and / or marital status) within the fiscal year for tax purposes.(The SS-5 form should be filed, even if you are keeping your own name.) along with an original proof of marriage. Social Security may take a while if you rely on the mail, so we always suggest you have another proof on hand. Therefore you may want to request two copies of your certified marriage certificate from the County Clerks Office. You can check with Social Security, you may be able to walk in and just show them your certificate and they might return it to you. Do not take a day off from work to visit any of the government offices or other business' until you have your certified copy of your marriage certificate. Change your name address if applicable on the following documents or offices:

Marriage License Information Requirements for obtaining County Marriage License 
 
1 ~ Both applicants must be present at     the County Clerks Office to request a Marriage License.
 
2 ~ Proof of age is required of both applicants. 

3 ~ The only identification documents which are accepted are:  
a) State issued Drivers license/State ID. 
b) Original or certified copy of birth certificate 
c) Military I.D. w/ photo and signature on it. 
d) A valid Government issued Passport and/or Government issued Identification Card..
 
4 ~ Legal age without a parents consent is 18 for both persons 

5 ~ If age 16 or 17, both parents or legal guardians of both applicants   must be present to sign the consent form.
 
6 ~ Written notarized consent is also allowed.
 
7 ~ If either party has been divorced, a certified copy of the divorce  decree or an original signed by the Judge must be presented.
 
8 ~ The marriage must be performed within  60 days after the license is issued. 

9 ~ There is a 1 day waiting period between application and the ceremony date. 

10 ~ The fee's vary from County to County. Cook County is currently at $35.00

11 ~ There is no blood test required in Illinois. 

12 ~ To obtain a copy of your Marriage License Certificate you must apply for it from the County Clerks Office. The cost varies by county. Cook County offers an online service for the request of a Certified License Certificate.

13 ~ Minister must file Marriage License with  County Clerks Office within 10 days of the  Wedding Ceremony.

14 ~ The marriage license must be obtained from the County in which the Wedding Ceremony is going to take place.
